What is an online form builder?
An online form builder lets you create a form in the browser, publish it at a link and collect the answers without building a website or writing code. People fill it in on a phone or computer, and the answers arrive in a results view you can read, filter and export.
Most businesses use forms for the same handful of jobs:
- Lead capture — demo requests, quote requests, "contact me" forms and event sign-ups.
- Feedback and surveys — customer satisfaction, post-event surveys and product research.
- Applications and registrations — job applications, course enrolment, membership sign-ups.
- Quizzes and assessments — scored quizzes, readiness checks and product finders.
- Operations — order forms, return requests and internal check-ins.
CardDeck Forms is the online form builder inside the CardDeck suite. It covers all of these with one editor, and because it shares a workspace with your cards, QR codes and contacts, a response doesn't end up in a separate tool you have to reconcile later.
Which question types does CardDeck Forms support?
CardDeck Forms supports 27 question types, and every answer is validated on the server, not only in the browser. That means a phone number field really holds a phone number, and a required field can't be skipped by editing the page.
Contact details
- Short text mapped to name or company, plus email and website (URL) questions.
- International phone numbers, checked against the country's numbering rules.
- Address fields that follow the country's format, so a UK postcode and a US ZIP code are both handled correctly, and a country picker.
Choices, scales and rankings
- Multiple choice, checkboxes, dropdowns, yes/no and image choice, where a picture explains the option better than words.
- Star ratings, opinion scales and Net Promoter Score (NPS) questions for satisfaction surveys.
- Ranking and matrix questions when you need people to compare several options at once.
Text, numbers, dates and consent
- Short and long text, numbers, dates, times and date-and-time.
- A consent question that records the respondent's agreement, plus statements and page breaks to structure long forms.
Files and signatures
- File upload for CVs, photos and documents. Files are checked by content, stored privately and only downloaded as attachments.
- A signature question, where the respondent draws their signature as part of the form. This is a form answer, not a contract-signing workflow.
If you are unsure which type to use, start from a template: each one already uses the right questions for its job.
How do conditional logic and branching work?
Conditional logic changes the form based on earlier answers, so each person only sees the questions that apply to them. In CardDeck Forms you set rules in the editor without code, and the rules are enforced on the server too.
You can:
- Show or hide a question — ask for a company name only if someone chose "For my business".
- Jump ahead — skip the pricing questions for people who only want support.
- Route to different endings — send high-intent leads to a "We'll call you today" message and others to a resources page.
- Calculate — keep a running total or score in a variable, for example the number of seats requested multiplied by a price.
- Recall — repeat an earlier answer later, such as "Thanks, Sam — which day suits you?"
Quizzes and scoring
Give each answer a score to build a quiz, a readiness check or a product finder. Endings can depend on the total, and correct answers are never sent to the respondent's browser, so they can't be found by viewing the page source.
Logic, calculations and scoring are included on Pro, Team and Business.
Where do form responses go?
Responses go to the form's results inbox, and, if you map the fields, into your CardDeck contacts as well. Nothing has to be copied between tools by hand.
The results inbox
Filter responses, star or flag the ones that need action, add tags, and act on several at once. Each response can be downloaded as a PDF.
Charts and drop-off
The summary view shows charts for each question and a funnel of views, starts and completions, so you can see which question makes people give up.
Contacts
Map the name, email and phone questions to contact fields. Each new response then creates a contact, or updates an existing one with the same email, and adds the tags you choose — "webinar sign-up", "quote request", "expo 2026". Your contact management view then shows form leads next to people who saved your digital card.
Exports and webhooks
Export responses as CSV or XLSX at any time. Exports are formula-safe, so a malicious answer can't run as a spreadsheet formula. For automation, a webhook sends each new response to your own system.
Can AI build a form for me?
Yes. The AI form builder drafts a complete form from a short description, such as "a quote request for a cleaning company with property size and preferred date". You get questions, answer options and settings you can edit before publishing.
AI requests use the monthly AI credits included in your CardDeck plan. If you connect your own AI key — OpenAI, Anthropic Claude, Google Gemini, Azure OpenAI, Mistral, Groq, DeepSeek, OpenRouter or another OpenAI-compatible API — AI is unlimited and billed by your provider. The AI form builder is available on every plan.
How does CardDeck Forms handle privacy and consent?
CardDeck Forms is designed for collecting personal data responsibly in the US, the UK, the EU and the UAE. You decide what to ask, and the tools help you keep only what you need.
- Consent wording — templates include consent text adapted by region, and you can add a required consent checkbox to any form.
- Erase on request — delete a respondent's data when they ask, in line with GDPR and UK GDPR rights.
- Retention — optionally delete responses automatically after a period you choose.
- Private files — uploads are stored privately and served only as downloads to your team.
- Spam protection — a hidden honeypot field and rate limits stop most bots without a puzzle for real people.
Ask only for what you will use. Shorter forms get more completions, and every field you drop is one less piece of personal data to protect.
How much does CardDeck Forms cost?
CardDeck Forms is included in every CardDeck plan, alongside Connect, Reviews and Screens. You pay for one plan and one set of seats, not for each app.
- Free — 1 form, 100 responses a month, 100 MB of file storage, both layouts and the AI builder.
- Pro ($8 a month, or $6 billed yearly) — unlimited forms, 5,000 responses a month, 5 GB of storage, logic, calculations, hidden fields, file uploads and no CardDeck branding.
- Team ($6 per seat a month, or $5 billed yearly, minimum 2 seats) — 25,000 responses a month, 25 GB of storage and save-and-resume.
- Business ($99 a month, or $79 billed yearly, with 15 seats) — unlimited responses, 100 GB of storage and white-label forms.
If a form reaches the monthly response limit, it shows a neutral "not accepting responses" message, and nothing already collected is deleted. See pricing for the full plan comparison.
